Services encompass Adult Day Health, Attendant Care, Caregiver/Recipient Training, Care Management, Chore Services, Environmental Accessibility Adaptations, Home-Based Supportive Care, Home-Delivered Meals, Personal Care Assistance, Personal Emergency Response Systems (PERS), Respite Care, Social Adult Day Care, Special Medical Equipment and Supplies, and Transportation.
Provides short-term home care for acute, crisis cases where no funding or support is in place for personal care to senior citizens or disabled individuals. Aides provide personal care such as: bathing, transfers, meal preparation, light housekeeping, medication reminders, and assistance with maintaining a safe environment.
Offers a New Jersey Department of Health and Senior Services Medicaid Waiver program for older adults, ages 65 and older, and disabled individuals that offers care alternatives for those who would otherwise be at risk of placement in a nursing facility.
