Application process
Call for information and/or screening. Screen for Community Services (SCS) is an automated tool that prescreens consumers for home and community based services. The automated system is built use the information given to automatically calculate a level of service needed. SCS can be conducted by telephone or in person.
Fees
JACC is not a Medicaid program and requires a cost share based on income only.

Salem County, NJ
Provides in-home and supportive services for individuals at risk of being placed in a nursing home facility. Services are intended to supplement and strengthen the capacity of caregivers and to delay placement in a nursing facility.

Offers assistance with Social Security, Medicare, Medicaid, Pharmaceutical Assistance to the Aged and Disabled. This includes income Tax and a wide range of other programs.
