Traumatic Brain Injury (TBI) Waiver | NJ Division of Medical Assistance and Health Services
- Diagnosis of acquired brain injury after their 21st birthday but prior to turning 65 years of age;
- Exhibit medical, emotional, behavioral and cognitive deficits;
- Have a rating of at least 4 on the Rancho Los Amigos Level of Cognitive Functioning Scale.

Provides in-home support to brain-injured individuals, who are currently in nursing facilities or who are in the community and at risk for placement in nursing facilities. Reintegrates into and maintains individuals in the community so that supervision is no longer needed or is minimized. Primarily serves ambulatory young adults with cognitive, behavioral, and physical deficits which require supervised and supportive care.
Offers health coverage to children, pregnant women, parents/caretaker relatives, single adults, childless couples, the aged, blind, disabled, and individuals qualified for long-term care services. Also provides comprehensive health coverage and a wide-range of services including: doctor visits, hospital services, prescriptions, tests, vision care, mental health care, dental, nursing home care and other healthcare services, depending on the person’s eligibility category.
