Diabetes Self-Management Education Classes (DSME) and training is a collaborative course through which people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. The course incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. DSME can be conducted on an individual and/or group basis, and is facilitated by a licensed health professional. The course covers topics such as: - Diabetes disease process & treatment options - Incorporating nutrition management, physical activity & utilizing medications - Monitoring blood glucose & using results to improve control - Preventing, detecting & treating acute & chronic complications - Goal setting and problem solving - Integrating psychosocial adjustment - Preconception care and management during pregnancy (if applicable)
Ensures that individuals with TBI (Traumatic Brain Injury) and their families have access to: information, referrals and advice; individual and family advocacy; legal representation; and specific assistance in self-advocacy. The program prioritizes reaching communities of color and underserved urban and rural populations.
Provides shelter, support, and education to homeless pregnant individuals. Services include: counseling, case management, career development, education, and life skills workshops.
Provides food assistance to residents of Camden County who are in need. This is done on the 4th Sunday of each month.
Provides programs including exercise activities, special events, health insurance counseling for Medicaid and Medicare concerns, monthly luncheon shows with entertainment, monthly international cooking programs, opportunities to join a senior club, computer literacy class at Ewing, information and referral Senior Health Insurance Program (SHIP), and office on aging.
Assists those interested in relocating to Camden County in locating, obtaining or retaining suitable housing. It is not an emergency housing program.
Provides food assistance to people in need. Supplies critical nutrition to hungry individuals and families.
Provides low-income and at-risk youth ages 16–24 with career technical training and education, enabling them to earn a high school diploma or equivalency, college credits, and industry-recognized certifications in over 100 career fields. Participants receive housing, meals, healthcare, a living allowance, and training at no cost during the 8–12 month program. After graduation, career transition counselors offer up to 21 months of support to help secure employment and independent living.
Offers career development services that help people with disabilities with find and maintain employment. This includes, but is not limited to: Career Planning, Job Development, Follow Along, Support to Employers, Worksite Accommodations. Also works with school districts to provide Transition Services to help students prepare for life after high school, whether it's finding employment or participating in a post-secondary educational program.
Provides various recreational activities. This includes abacus practice, art classes, bible studies, book club, chess club, drop-in center, gardening, knitting, and music appreciation.
Offers a variety of activities including crafts, line dancing, computer classes, self defense courses, and bingo. A lunch time meal is served daily. The centers are open during the day and occasionally offer special activities and events during the evening.
Provides various care services to individuals in the community. Services include: long-term care, short-term rehabilitation, and respite care. Specialized services include: Huntington’s Disease care; subacute rehabilitation; physical, occupational and recreational therapy; IV therapy; and wound care. Also provides furnished rooms, meals, and recreational activities.
Deliver comprehensive, coordinated primary care through an FQHC model, offering internal medicine, pediatrics, women’s health, dental, family planning, and behavioral health services through an interdisciplinary team in a culturally sensitive, patient-centered setting.
Assists applicants with various benefits including retirement, disability, survivors, SSI, and Medicare. Applicants should apply for retirement benefits at least 3 months before retiring, and disability payments are for those with severe impairments hindering work or leading to death. Eligibility for disability under SSI depends on work history and financial resources. A replacement Social Security card can also be obtained in-office. Check the website for online services.
