Intensive case management focuses on developing a single, coordinated care plan for pregnant/postpartum individuals, their children, and families. Recovery support specialists provide non-clinical assistance and recovery supports.

Assisting families dealing with mental health conditions through various services:

1. Provide Information and Referral: Offer directories, articles, brochures, and a library of resources on mental illness, treatment, housing options, and legal/financial programs. Staff is available to discuss and explore ways to meet specific client needs.

2. Offer Consultation and Education: Provide individual or family guidance with mental health professionals over the phone or in person. Deliver an objective, supportive perspective for handling situations or managing the illness. Consultation available at the agency or home with flexible hours.

3. Facilitate Support Groups: Conduct ongoing support groups for families coping with mental illness. Create an opportunity to share and problem-solve with others facing similar situations. Guest speakers cover topics like estate planning, medication, relapse prevention, and treatment resources.

4. Provide Respite Care: Offer non-emergency relief for family members seeking a break from caregiving responsibilities.

Provides intensive care management to individuals who are pregnant or up to one year postpartum and are using or have recently used alcohol or drugs. 

Aids clients in accessing community-based programs and services, developing personalized plans to meet individual needs. By crafting service plans, support coordinators identify goals, needs, and natural supports within the family and community.

A variety of professional home care needs. This includes nurses, certified home health aides, physical, occupational and speech therapists, Medical Social Workers and dieticians.    

Provides routine, non-medical assistance to adults with disabilities who are employed, involved in community volunteer work, or attending school and are not eligible for Medicaid. Personal assistants help with tasks such as: light house keeping, bathing, dressing, preparing meals, shopping, driving, or using public transportation.

Provides free, confidential, and non-judgmental pregnancy support services that offer emotional support, information, and practical assistance to individuals facing unplanned pregnancies. Connects individuals to pregnancy testing, referrals for medical care, housing, legal and financial resources, counseling, and community services; supplies maternity and baby items; and delivers ongoing support before and after birth to help individuals develop safe and realistic plans for themselves and their children.

Provides a variety of recreational, educational, cultural, and wellness programs for adults 60+, including group exercise, arts and crafts, social clubs, holiday events, health screenings, transportation support, and special interest activities.

Provides a parent support group for families with a child with a mental health issue. There is limited childcare and transportation available. Refreshments are served.   

Provides free legal advice for merchant mariners and port chaplains on work related issues. Offers educational and other support programs for international seafarers, as well as domestic mariners.
