Provides primary health care services to individuals in the community. Adult services include: preventive care, screenings, immunizations, and specialized evaluations. Children and adolescent services include: well visits, immunizations, screenings, and comprehensive care. Obstetrics and gynecology services include: pregnancy care, and general gynecological care. Also provides infertility diagnostic care and counseling.

Promotes and supports exclusive breastfeeding, conducts health and nutrition screenings for early identification or treatment of risk factors, refers participants to essential health care and social services, conducts personalized nutrition/health counseling in individual and peer/group sessions, and issues food vouchers with essential nutrients to address deficiencies or lacking in their diets.   

Provides subsidized independent living community for low-income seniors and individuals with disabilities . The community offers one and two bedroom units, with individually controlled heat and air conditioning units and fully equipped kitchens.

Provides an evidence-based lifestyle change program for preventing type 2 diabetes. These year-long classes help participants make real lifestyle changes such as eating healthier, including physical activity into their daily lives, and improving problem-solving and coping skills.

Provides free or low-cost health care that may help pay for medical bills, doctor visits and prescriptions for certain low-income individuals or families. NJ Family Care covers doctor visits, eyeglasses, hospitalization, lab tests, x-rays, prescriptions, regular checkups, mental health, and dental. 

Offers financial assistance with heating and energy costs to families and individuals experiencing financial distress.

Offers an individualized plan of care to meet each individuals needs who attend on a scheduled basis. Services provided include social services, recreational activities, nutritious meals, and coordination/referral for services with a licensed provider. 

Assists low-income individuals in weatherizing their homes. Improves their heating system efficiency, conserving energy, and improving health and safety standards.

Provides assistance with applying for health coverage. Trained and certified staff members can help individual consumers, small businesses and their employees understand health coverage options, complete eligibility forms and enroll in a plan through the health insurance marketplace. Medicaid and the Children's Health Insurance Program (CHIP) is available to those who qualify.   

Provides food assistance for individuals in need.

Offers a variety of support groups for senior adults. The topics would include; caregivers, bereavement, retirement, Holocaust survivors   

Provides persons with disabilities with a personal assistant who can help with services such as dressing, bathing, housekeeping, meal preparation, correspondence, mobility, driving, and/or use of public transportation. The service is for those going to work, school and/or remaining active in their community. Care can be provided to 40 hours per week.  

Administers voter registration and election services by overseeing polling locations, vote‑by‑mail and early voting processes, election staffing, and ballot administration to ensure fair, accurate, and accessible elections for Somerset County residents.

Assists veterans and their families with VA claims, pensions, compensation, state benefits, and questions about VA entitlements and services.

Provides Community Support Services (CSS) to assist individuals with mental illnesses. Manages symptoms and maintain roles in employment, education, housing, and social settings.

Provides job search/job readiness training, HSE (high school equivalency) preparation, and employment placement assistance. Counselors work with individuals to help develop and achieve their goals.
