Support coordinators design, approve, coordinate, and monitor a comprehensive support plan for individuals throughout New Jersey who receive services through NJ DDD Support Program. Our method utilizes a blend of professional and natural resources, such as family, neighbors, and community resources that makes the most of each person's assigned DDD budget.
Provides behavioral health care program that provides psychotherapy and counseling for people with behavioral, emotional, psychiatric and substance abuse concerns. Provides emergency baby supplies including diapers, wipes, formula, baby food, and baby toiletry items to mothers of newborns. Clients can only visit a center once every two months.
Provides health insurance information and assists with comparing or enrolling in health insurance. Navigators provide free and impartial information to ensure the best options to meet the specific needs of individuals and families.
Provides outreach, assessments and case management services to seniors and individuals with disabilities. Referrals may include information for adult day care programs, support groups, transportation, home care, respite, senior centers and home delivered meals. Case manager can complete an home visit and, along with the resident and family, identify the needs and create a service plan to meet those needs.
Services are for adults (18 or older) who are not linked with other mental health services and who need immediate mental health services. This is for consumers who can remain safe in the community and do not require emergency services.
Assists defense teams in cases involving battered women by providing technical assistance on civil and criminal law matters, including identifying defense strategies, offering relevant case law and litigation materials, locating expert witnesses, identifying support networks for victim defendants, and suggesting additional sources of information and assistance. Additionally, offers information on promising practices for responding to domestic violence, accessible through a toll-free telephone line. Does not offer direct representation.

Through mentorship, they set goals, make sound choices, and plan for self-sufficiency by securing employment. Provides individual and family counseling for past or present issues, aiming for successful permanent housing, healthy independent living, budgeting skills, and community enhancement.
