Enables volunteers, ages 55 and over, to remain physically and mentally active by partnering them with children with special needs. Volunteers are trained to work with children in order to improve their physical, mental, emotional and social development. Children in the program range from ages 1 through 12.
Diabetes Self-Management Education Classes (DSME) and training is a collaborative course through which people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. The course incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. DSME can be conducted on an individual and/or group basis, and is facilitated by a licensed health professional. The course covers topics such as: - Diabetes disease process & treatment options - Incorporating nutrition management, physical activity & utilizing medications - Monitoring blood glucose & using results to improve control - Preventing, detecting & treating acute & chronic complications - Goal setting and problem solving - Integrating psychosocial adjustment - Preconception care and management during pregnancy (if applicable)

Services encompass physical, emotional, and spiritual support, assistance with personal care, medication for symptom management, medical equipment for home care, short-term respite care, availability 24 hours a day, 7 days a week, registered nurse by phone, and bereavement follow-up and workshops. One such workshop, "Understanding Grief," is open to the community, providing guidance for those newly bereaved or anticipating a loss at no cost.

Support encompasses prescription aid (up to $1500 annually), home health care (up to 6 hours per week), social adult day care (up to 2 days per week), medical adult day care (up to 1 day per week), home-delivered meals (1 meal per weekday), barrier-free home modifications (with a $4500 lifetime maximum), and durable medical supplies and equipment (up to $600 annually). Medical equipment and supplies include items like diapers, colostomy bags, diabetic supplies, canes, crutches, walkers, commodes, bath chairs, and similar products not covered by Medicare.

USF Overview
Offers monthly credits on electric and/or gas bills to reduce ongoing energy costs. Designed for households that spend a significant portion of their income on energy. Helps prevent service interruptions and promotes long-term affordability.
Offers various treatments of behavioral health issues such as anxiety and stress, depression, and Post Traumatic Stress Disorder. This program includes treatment options such as individual counseling sessions, group counseling sessions and psychiatric appointments. Language translation services available upon request.
Supporting individuals, our After School Program offers respite for children under 18 with developmental disabilities after their regular school day. For adults, our After Care Program offers respite after their work or training day, fostering a supportive environment.
Provides supportive services for veterans and their dependents/survivors, including: over the phone counseling, filing claims or obtaining military history, transportation to and from a Veterans Hospital, and assistance obtaining markers and making arrangements for burial in the County Veterans Cemetery.
