Provides information and referral services to people with disabilities and their families who are seeking help locating appropriate resources in their community. The program is responsible for overseeing the Personal Assistance Services Program that is designed to help people with disabilities live as independently as possible.

Also provides group services for boarding homes and community-based locations, support, education, recreation, socialization, and advocacy; multicultural outreach to underserved populations and advocacy, support; and training for consumers of mental health services through advocacy in community-based mental health and community boards, committees, and task forces.

Also provides various care management services: medical visits, family member consultations, prospective care community visits, reviews of long-term care policies, and assistance with discharge planning from a hospital or rehab.

Connects patients and practitioners to behavioral health services to assess needs, determine appropriate care levels, verify insurance and arrange pre-certification, schedule appointments, and connect to information and referrals to various programs. Available referrals include: behavioral health programs, adult and adolescent services, substance abuse programs, detox unit, in- and out-patient psychiatric care, and psychiatric emergency services.
