Goals encompass teaching self-management, promoting community integration, preventing hospitalizations, and enhancing quality of life. Services include 24/7 PACT team availability, personalized home-based support, medication management, crisis response, career counseling, and life skills training. PACT aims for positive outcomes like employment, education, supportive networks, improved family relations, and a substance-free life. Lifelong services are available as needed.
