Eligibility
Adults residing in Gloucester County, age 18 or older, diagnosed with a severe and persistent mental illness.
  • Within the last 18 months, individuals must have had either:
  • Two or more state hospitalizations, or
  • One state hospitalization with one or more other psychiatric hospitalizations, or
  • One state hospitalization with multiple-screening episodes

Enhances psychiatric stabilization, improves the quality of life, and prevents unnecessary hospitalizations for Severe and Persistent Mentally Ill (SPMI) individuals with a history of hospitalization through intensive outreach support services. The program provides continuous care, regardless of the client's living situation, offering services such as symptom assessment, medication management, crisis intervention, intensive case management, support for daily living, assistance with recreational and social activities, substance abuse counseling, vocational and employment services, and support for family and social networks.
