Assistance includes NJ Family Care and Medicaid enrollment, as well as special programs for Medicaid-qualified individuals such as Long-Term Care expense assistance, hospice programs, waivers for AIDS and Brain Injury patients, and non-emergency medical transportation.
Provides community-based long-term care through the Community Care Program for the Elderly and Disabled (CCPED), serving adults age 18 and older who meet nursing facility level-of-care criteria and wish to remain in the community. Coordinates case management and in-home and community supports, including personal care, adult day health, respite, meals, medical escort, and home accessibility adaptations, for individuals with chronic illness, physical disability, or functional limitations.

Provides in-home support to brain-injured individuals, who are currently in nursing facilities or who are in the community and at risk for placement in nursing facilities. Reintegrates into and maintains individuals in the community so that supervision is no longer needed or is minimized. Primarily serves ambulatory young adults with cognitive, behavioral, and physical deficits which require supervised and supportive care.
