Assists low-income individuals in weatherizing their homes, improving their heating system efficiency, conserving energy, and improving health and safety standards in the home.

A collaborative course through which people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. The course incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. DSME can be conducted on an individual and/or group basis and is facilitated by a licensed health professional. The course covers topics such as: - Diabetes disease process & treatment options - Incorporating nutrition management, physical activity & utilizing medications - Monitoring blood glucose & using results to improve control - Preventing, detecting & treating acute & chronic complications - Goal setting and problem solving - Integrating psychosocial adjustment - Preconception care and management during pregnancy (if applicable).  

Delivers a nutritionally-balanced meal and assurance check each weekday to home-bound seniors. Two cold meals for weekend consumption are provided for participants on Fridays.  

Provides education on Teen Pregnancy prevention, and healthy relationships. Also Assists with counseling and relationship issues. 

Offers several employment and educational services for women in the community. Programs include: job training; basic, work, computer, and life skills training; GED; nutrition assistance; food handling; and domestic violence services.

Provides free legal advice and counsel to women who are victims of domestic violence. Partners may provide advice and counsel regarding temporary restraining orders, child support, visitation, custody, divorce, and final restraining order hearings. All services are provided over the phone on a case-by-case basis.

Provides wraparound support services for recently-incarcerated individuals. Services include: addiction and behavioral health treatment, sober transitional housing, Medicaid registration and healthcare linkage, assistance with Motor Vehicle Commission identification, legal services, mentoring through faith-based and professional networks, and support for training and employment.

Offers beginner, intermediate, and advanced English as a Second Language (ESL) classes. Classes are held twice weekly for 3 hours each over a 10-week session. Morning and evening sessions are available.

Provides experienced, quality, end-of-life care to patients and their families. Operating in-home or at the hospital's Inpatient Center, the HospiceCare's team provides specialized care to relieve pain and control symptoms while enhancing comfort and quality of life for terminally ill persons and their families during the final stage of any disease.   

Provides therapy and medication services to individuals with limited resources. Services include: individual, family, group, and marital therapy for anxiety, depression, family conflicts, and everyday living challenges. After an initial psychiatrist interview, individuals, ages ten and older, are referred to therapists in private offices. Also provides, medication services at the Montclair location.

Offers a free membership that includes a resource directory, newsletters, allows participation in quarterly meetings, and elections offering information, education, and support to women interested in or currently breastfeeding.

Assists senior citizens, ages 62 and older, with home equity services. Provides access to programs through counselors from the Agency for Housing and Urban Development (HUD).

Provides food assistance for individuals in need.

Operates a designated county Crisis Screening Center for individuals experiencing an urgent mental health crisis. Utilizes a mobile response team to provide services both at home or on-site of a crisis.
