Provides specialized short-term rehabilitation and long-term care for patients with brain injuries or on ventilators. Services include: extended recovery brain injury care, ventilator care, subacute rehabilitation, cognitive rehabilitation therapy, respite care, and a residential unit. In-home physical, occupational and speech therapy to regain abilities diminished or lost as a result of illness, injury or advanced age. Therapists conduct client assessments and design individualized therapy regimens for their patients, consulting with their patients' personal physicians and other VNA professionals as needed, and adjusting the frequency and difficulty of the therapy sessions as their patient progresses and nears discharge from the in-home program.

Facilitates a structured rehabilitation program for individuals with brain injuries, aiming for community independence. Tailored therapy sessions mimic real-life situations, aiding reintegration. Patients can access services up to six hours a day, five days a week. Trained professionals offer educational, vocational, and functional activities.

Specialized services include: Huntington’s Disease care; subacute rehabilitation; physical, occupational and recreational therapy; IV therapy; and wound care. Also provides furnished rooms, meals, and recreational activities.

Operates an inpatient rehabilitation center for individuals with diverse disabilities. Assists patients in regaining essential skills for independent living, such as walking, bathing, and speech. Also provides accommodations for special needs patients, including dialysis, transfusions, TPN, PEG tubes, and tracheotomies.
