Prioritizes and assists homeless individuals and families in obtaining permanent housing, serving as a point of exit from homelessness. Consumers must satisfy the criteria for homelessness as defined by the U.S. Department of Housing and Urban Development (HUD) and are referred by the PATH program. Individuals may also be referred by homeless shelters and the Monmouth County Social Services Homeless Intake.
