Application process

In order to register for this program, interested individuals should call the Middlesex County Office of Aging and Disabled Services for information. An intake form will need to be completed along with a liability release form. Updating the data on the card is the sole responsibility of the applicant. All data must be kept accurate and up-to-date.

Updating the information is the responsibility of the cardholder or a designated person. While useful, the card does not replace the necessity for additional information and education on medication safety and consulting healthcare professionals for concerns.
