Eligibility
Open to youth up to age 21 with NJ Medicaid/NJ FamilyCare, behavioral health needs, and qualifying chronic medical conditions in the service area.

NJ Medicaid/NJ FamilyCare information, insurance information, medical records of qualifying chronic health conditions, screening test, Photo ID and demographic information.

Enhances traditional Wraparound care management by coordinating physical health, behavioral health, wellness education, hospitalization follow-up, medication monitoring, and communication among medical and behavioral health providers
