Eligibility

Individuals with Metastatic Breast Cancer, or who are currently in treatment for breast cancer which was diagnosed in the past 24 months. Individuals must also:- have a household income at or below 300% of the federal poverty level; - have not received an award in the past 12-months and;- be residing in the US or a US territory.

Required documents

Documents verifying date of breast cancer diagnosis, cancer type, cancer subtype, and current stage medical documentation on official letterhead from the past 12 months containing the patient's name, date of birth, current diagnosis and stage, and confirmation of active treatment proof of household income banking information

Description

Provides financial assistance for individuals currently undergoing breast cancer treatment or living with Metastatic Breast Cancer. Awards may be used for treatment-related expenses, such as: housing, utilities, transportation, child and elder care, food, side-effect management medication, oral treatment medication, medical equipment, lymphedema care and supplies, palliative care, and home health care.
