Eligibility
Open to Union County residents who are HIV positive. HIV testing open to all.
Required documents
ID, insurance information, medical/dental records, photo ID, proof of address, proof of application for other assistance.

Application process
Please call or walk-in to register and begin the intake process. Applicants should bring the following:
  • Two (2) forms of ID, one with photo
  • Documentation of HIV status
  • Proof of address
  • Medical insurance card
  • Entitlement award letters (SSI, SSDI, Welfare)
Those who are part of a Health Management Organization (HMO) will need a referral form from their primary care physician.
Fees
The program accepts Medicaid and most major insurances.

Provides HIV testing, primary medical care, case management, and prescription drug assistance to HIV-positive individuals from Union County at Trinitas Regional Medical Center. Delivers comprehensive medical care by infectious disease specialists, gynecology referrals, medication management, and coordination of inpatient and outpatient care. Administers pneumonia, flu, and hepatitis vaccines, screen for Tuberculosis, and offer on-site care Monday through Friday. Provides education on medications, treatments, and risk reduction. Case management includes counseling, referrals to substance abuse and psychiatric services, financial and benefit counseling, food and housing assistance, prescription aid, and support for permanency planning of minor children with ongoing follow-up.
