Eligibility

Treatment destination must be one of the participating institutions listed on the website. In addition, the patient must be: 21 years old or younger, currently receiving treatment for childhood cancer (diagnosed before 18th birthday), and have a family income no greater than $100,000.

Application process

Online applications must be submitted by a social worker or member of the oncology care team. Expect a response within one business day. Travel For Care Program Application Form, and Approve applicant request form

Fees

None. Maximum benefit is $4,000 per family.

Description

Offers assistance to families who face the need to travel for childhood cancer treatment. Transportation and lodging accommodations are available to one caregiver who meet the criteria. Funds are not intended to be the sole source of travel assistance, and can only fund out-of-pocket, upfront travel expenses (no reimbursements). Assistance is only available for travel within the US and Canada.
