Application process

Residents of Mercer County may file an application for Medicaid at the Mercer County Board of Social Services. Based on need, applications can be taken at home, in a hospital or in a nursing facility.

Provides customized care to individuals, ages 55 and older, who live in Mercer County and meet the nursing facility level of care established by Medicaid. Services include, but are not limited to:

  • Adult day care
  • Assisted living
  • Nursing homecare
Individuals receive a customized care plan that is delivered by a coordinated interdisciplinary team.
