Eligibility

Open to consumers 3 years of age and older. Open to residents of Clifton, Passaic, East Rutherford, Garfield, Little Falls, Lodi, Lyndhurst, Secaucus, Wallington, and Woodland Park.

Required documents

ID, Proof of insurance, and Proof of income for sliding scale participants.

These services include community-based geriatric services, family counseling, crisis intervention, case management, child-centered mental health services, school consultations and domestic violence services.
