Supports eligible residents under the Division of Developmental Disabilities (DDD) by offering Support Coordination services. Coordinates individualized service plans, identifying needs, goals, and service providers. Facilitates access to services, completes necessary paperwork, and ensures ongoing service delivery. Acts as a liaison, advocate, and case manager, supporting participants and their families throughout the process.
Provides paid work opportunities to developmentally disabled consumers. Assists with connecting consumers with apartment, group home, and residential living services. Daytime care facilities available for the more developmentally delayed, and developmentally disabled adults.
