Application process
Call to schedule an appointment. A physician's referral or an insurance's referral will be required before registering. Individuals without a referral can receive assistance in obtaining one.

Description
Empowers individuals with diabetes through collaborative Diabetes Self-Management Education Classes (DSME), fostering the acquisition of knowledge and skills to modify behavior and successfully self-manage the disease. Guided by evidence-based standards, the course addresses the unique needs, goals, and life experiences of each participant. Facilitated by licensed health professionals, DSME, available in both individual and group formats, covers diverse topics, including the disease process, treatment options, nutrition management, physical activity, medication use, blood glucose monitoring, acute and chronic complication management, goal setting, problem-solving, psychosocial adjustment, and preconception care during pregnancy.
