Eligibility
- Veterans of all wars who served on active duty and were honorably discharged
- Qualified National Guard/Reserve Retirees
- Veteran's spouse/widow/widower (must be at least 50 years of age and must have been married to the veteran for a period of not less than 10 years)
- Gold Star Parent (parents of members of the military who were killed in action during a period of war)

Application process
Complete the application found on the website. It will not be saved, so it must be printed out. A physician or financial institution must complete where indicated. The document must be notarized.

- This facility is not in network with any HMO/Medicare Advantage Card and does not participate in Medicaid program.

Offers rehabilitative and supportive services, including occupational, physical, speech, and recreational therapy, as well as dental, podiatric, dermatological, and ophthalmological care. Includes orientation for new residents, family support groups, educational programming, and individual and group counseling.
