Eligibility

Open to developmentally disabled individuals, ages 21 and older. Applicants must be eligible for services from the New Jersey Division of Developmental Disabilities (DDD) and be eligible for Medicaid. In addition, applicants must be considered medically fragile in some capacity and require medical supports.
