Application process
Complaint forms can be completed online and submitted on the Consumer Affairs website.
Form can be downloaded, completed, and mailed to the address below. If downloading a form is not an option, a form can be mailed by calling 609-826-7100 to request a copy.

To find complaint forms online, please follow the below steps:
1. Go to http://www.njconsumeraffairs.gov
2. Select the "Boards and Committees" tab at the top of screen
3. In the middle column, select "Medical Examiners"
4. On the far-left side, near the bottom, select "File BME Complaint"
5. Complete all the required fields and press "Submit" at the bottom on the form
