Individuals with diabetes and their families, with particular focus on those who are newly diagnosed.

Application process
A physician's referral will be required before registering. Those without a referral MAY receive assistance in obtaining one.
Please call to schedule an appointment or to register for one of the classes.
Fees
$120 per class, or $600 for the entire course. Most medical insurances, including Medicare, are accepted. Amount of coverage varies by insurance plan. Medicare covers 10 hours of training. Please call for more information.

A collaborative course through which people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. The course incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. DSME can be conducted on an individual and/or group basis and is facilitated by a licensed health professional.
