Offers recovery support services to individuals treated with Narcan (Naloxone) for opioid overdoses. Services are initiated by hospital providers and include: Support for individual’s post-hospital discharge through follow-up calls and services. Linking individuals to treatment and recovery services. Community education on overdose prevention and obtaining Naloxone. Additional resources for individuals and their families interested in recovery. Upon hospital discharge, Recovery Specialists introduce individuals to the program, providing 8 weeks of follow-up support. Navigator meets with the individual within 24 hours of discharge, facilitating linkages to recovery resources, including treatment, shelter assistance, and transportation, with the goal of keeping people engaged in recovery.
