Open to adults and adolescents experiencing substance use disorder in the service

Required documents

Photo ID, insurance card, Medicaid card, proof of residency, Court documents or referral forms, and medication list or medical records if receiving behavioral health treatment.

Mon - Thurs 8:30 am - 8:30 pm, Fri 8:00 am - 4:30 pm Evening group sessions may be scheduled for outpatient treatment.
