Offers access to pre and post-natal health care and substance abuse reduction or abstinence support. The program provides support to keep mothers and children together and strengthen family relationships. Services will combine intensive case management, wrap around services and recovery supports, focused on developing a single, coordinated care plan for pregnant/postpartum women, their infants and families.
