Application process
New Jersey residents who receive the COVID-19 vaccine will automatically be enrolled in the New Jersey Immunization Information System.

Shifts inclusion into the New Jersey Immunization Information System from an opt-in to an opt-out program for residents choosing to receive a COVID-19 vaccine. Automatically enrolls this group into the system, allowing them to opt-out 30 days after the public health emergency ends. For those born on or after January 1, 1998, enrollment is automatic unless opting out, while those born before can choose to opt-in.
