Available to New Jersey residents who have hearing loss and a combined household income no great than 400% of the federal poverty level.

Required documents

Application and hearing loss verification (by either: attaching an audiogram signed by a licensed audiologist OR obtaining a signature on the application by a licensed audiologist or physician verifying applicant’s hearing loss).
