Application process
Call to schedule a reservation. All calls must be placed at least 2 business days before the transportation will be needed.

Operates as the state's medical transportation broker and can arrange non-emergency medical appointments transportation for Medicaid recipients throughout the state of New Jersey. Modivcare will also pick up patients who require the assistance of canes, wheelchairs, oxygen, IV drip, and other medical devices.
