Provides supportive services that help adults living with serious mental illness live independently in the community. Assists individuals with securing and maintaining affordable permanent housing, accessing employment and educational opportunities, and reducing psychiatric hospitalizations. Offers housing search and move‑in assistance, mental‑health and medication education, employment support, services for co‑occurring conditions as needed, supportive counseling, and around‑the‑clock on‑call support.
