Open to individuals who are pregnant and struggling with a substance use disorder with income at or below 350% of the Federal Poverty Level.

Provides intensive case management, wrap around services and recovery supports, focused on developing a single, coordinated care plan for pregnant/postpartum women who are using or have recently used alcohol or drugs (including prescription narcotics). Services are provided during their pregnancy and for up to one year after a birth event.
