Application process
Call to schedule an appointment. A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one.
Fees
Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. There is a prorated rate option available for uninsured clients. For Charity Care, call the hospital for more information on how to apply.

Diabetes Self-Management Education (DSME) classes help individuals with diabetes learn how to manage their condition effectively through evidence-based practices.
