Application process
Call to schedule an appointment for an initial assessment. A physician's referral will be required before attending the assessment.
After the assessment, a diabetes educator will determine if one-on-one counseling or a group session would be best Sted for the patient.
Fees
Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. Charity Care and/or payment plans may be available through the hospital. Please call for information.

Empowers individuals with diabetes through collaborative Diabetes Self-Management Education Classes (DSME) and training, guiding them to modify their behavior and successfully self-manage the disease. Facilitated by licensed health professionals, the course covers evidence-based topics such as the disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, prevention and treatment of acute and chronic complications, goal setting, problem solving, psychosocial adjustment, and preconception care and management during pregnancy if applicable. Classes can be conducted individually or in groups.
