Eligibility
Open to individuals living with HIV/AIDS who are in need of treatment to address their substance abuse and mental health disorders and any other chronic illnesses associated with HIV/AIDS.
Required documents
Birth certificate, driver's license, insurance information, Medical clearance, prescription, proof of address, proof of citizenship, psychiatric/psychological assessment, Social Security card/number.

Fees
An administrative fee applies but is revised on a case-by-case basis. The agency will not deny anyone of treatment if they cannot afford the fee. Call for more information.
