Provides family members and caregivers of intellectually and developmentally disabled individuals with the resources and referrals necessary to navigate through the system of care. Provides one-on-one support and advocacy to help bridge the gap between service changes and families. Family members and caregivers can submit a Problem Intake Form for assistance and advocacy with concerns such as service application or eligibility, difficulties in accessing services, educational matters including Individualized Education Programs (IEPs), transitioning from school to adult life, and determining SSI/Medicaid eligibility.
