A component of the Aging and Disability Resource Connection (ADRC) which offers information, facilitates referrals, and provides linkage to essential services to individuals (60 or over) who either have a disability or are caregivers to individuals over 60. Staff assess needs and coordinate community-based services to enhance the quality of life of both the caregiver and the care recipient.
