Provides permanent, supportive housing for individuals and families experiencing homelessness who have HIV/AIDS, serious and persistent mental illness, and/or substance use disorders. Offers onsite staff support 24 hours a day, 7 days a week, crisis intervention, mental health and substance use services, home health care, benefits assistance, nutrition education, food preparation support, vocational rehabilitation, life skills training, family support, and case management with referrals to community services.
