Eligibility
Open based on income if 135% or less than the federal poverty guidelines.
Participation in a qualifying federal assistance program:
- Supplemental Nutrition Assistance Program (SNAP), formerly known as Food Stamps
- Medicaid
- Supplemental Security Income (SSI)
- Federal Public Housing Assistance (FPHA)
- Veterans Pension and Survivors Benefit
- Tribal Programs (and live on federally-recognized Tribal lands)
Required documents
Application/registration forms, income tax return, Medicaid card, proof of application for other assistance, Proof of benefits (i.e. EBT, TANF, GA), proof of household income, proof of income eligibility, proof of retirement status, Section 8 housing voucher.

Call or visit the website for an application form. Interested applicants should mail the completed application form, along with the required documents.
