Offers a continuum of residential services for psychiatrically disabled consumers with a history of hospitalization, The CHAMP Program and The Partnership Program, along with two transitional group homes and long-term residences, offer daily support services. These encompass 24-hour supervision, psychiatric on-call assistance, evaluation, medication monitoring, nursing assessments, personal care training, assistance with daily living, recreational programs, and referrals to community services.
