Provides assessments, direct services, and referrals to community programs. Offers assistance with Social Security Disability Insurance, Supplemental Security Income, Medicare, Medicaid, Managed Long Term Services and Supports, food assistance programs, Pharmaceutical Assistance to the Aged and Disabled, and Meals on Wheels. Also helps individuals, families, and caregivers access medical equipment, home modifications, and home care services.
