Eligibility
Individuals must meet the following criteria:
- Must be currently receiving Medicaid
- Must have at least 1 needed, skilled service (blood pressure/glucose checks provided daily)
- Require at least limited assistance in a minimum of 2 ADL's or supervision/cueing in at least 3 ADL's and exhibit problems with short-term memory and with following multi-task sequences and daily decision making

Provides various medical services to adults. Services include: on-site monitoring of vitals, glucose, and medications; physical therapy; nutrition management; podiatry services; therapeutic recreation; and transportation assistance. Transportation is provided for Newark and the surrounding areas. Registered nurses are available on-site.
