Provides a one-stop shop, serving as a community-based, family-centered neighborhood gathering place providing support, information, and services. Core services include access to child, maternal, and family health services information, linkage to public health insurance programs, referrals to local health care services, development of family stability plans, economic self-sufficiency and employment-related services, income security services, job readiness training, information and referral services, life skills training, housing-related services, parent education, parent-child activities, advocacy, and home visiting in line with the approved model by the division of prevention and community partnership.
