Required documents
Proof of address (i.e. utility bill, license, etc)
Proof of the number of household members (Social Security Cards)
And ONE of the following:
Proof of participation in any of the following (Dated in the past 3 months):
Temporary Assistance to Needy Families (TANF)
The Food Stamp Program
Supplemental Security Income (SSI)
Women, Infants, and Children (WIC)
Medicaid
Medicare
US Veteran ID
OR
Proof of income (Dated in the past 3 months):
Current paycheck stub
Bank statement
Tax 1040 form (Dated the last year)
A letter from employer stating gross wages
Copy of unemployment letter or check stub
Copy of benefit letter from welfare
Copy of award letter from Social Security Administration
Eligibility Guidelines for Additional Services:
Baby Products for PARENTS/GUARDIANS of children under care in SAME household:
1-Baby Formula: Need Birth Certificate, Up to 2 years old
2-Diapers: Need Birth Certificate, Up to 4 years old
3-Adult Diapers: Proof of disability or being a senior citizen
4-Feminine Hygiene Products: For females in the household that would use the products.
