Eligibility
Available to eligible households of children cared for by grandparents, relatives, or friends. Individuals may qualify if they can provide relation by blood, marriage, legal guardianship, or the child has lived in the home for at least one year.

Proof of ID/relationship, court records (custody), proof of income, proof of address, birth certificate, and social security cards
