Eligibility
Open to adults age 18 and older who can self-direct services and are employed, preparing for employment, attending school, or volunteering at least 20 hours monthly in the service area.
Required documents
Photo ID, completed pre-application and screening, proof of residency, proof of income, proof of qualifying program activity, physician’s certification, and statement of agreement.
